We will be performing tooth gem services, which include the attachment of decorative gems or jewels to your teeth. The procedure involves the application of dental adhesive to secure the gem in place. Please note that this service is for cosmetic purposes only and does not involve any dental or medical treatment.
The Details
While tooth gem application is generally considered safe, it is important to understand that there are certain risks and limitations associated with the service. These may include but are not limited to:
Potential tooth or enamel damage during the application or removal process.
Possible allergic reactions or sensitivities to the dental adhesive or materials used.
Temporary discomfort or sensitivity in the area surrounding the tooth gem.
The potential for the gem to become loose or dislodged, requiring reapplication or removal.
The need for additional dental treatment if any complications arise during or after the procedure.
The possibility of discoloration or staining of the tooth surface underneath the gem.
Tooth gems may only be applied to natural tooth surfaces and will not adhere to artificial tooth surfaces.
Client Responsibilites
By signing this waiver, you agree to the following responsibilities:
Providing accurate medical and dental history, including any known allergies/sensitivities, the use of a retainer, etc.
Following post-application instructions and maintaining proper oral hygiene.
Contacting a qualified dental professional in case of any concerns or complications.
Accepting the risks involved and releasing Aces Over Eights Tattoo and employees of Aces Over Eights Tattoo from any liability arising from the tooth gem.
Informed Consent
By signing this waiver, you confirm that you have been given adequate information about the tooth gem services, including the risks involved, and have had the opportunity to ask any questions to your satisfaction.
You understand that the decision to proceed with the tooth gem application is entirely voluntary, and you assume full responsibility for any outcomes resulting from the procedure.
Release of Liability
In consideration of the tooth gem services provided by Aces Over Eights Tattoo, you agree to release, waive, and discharge Aces Over Eights Tattoo and its representatives from any
and all claims, liabilities, damages, or expenses arising out of or in connection with the tooth gem application, including but not limited to personal injury, loss, and property loss.
